OSU Minimum Web Accessibility Standards (rev. 06/30/2004)
Annual Report Template -- Excel Version
Note: This document represents a sample template of the data necessary to
meet the reporting requirements stipulated in the OSU Web Accessibility Policy
(rev. 06/30/04) [http://ada.osu.edu/resources/WebPolicies.htm] . Reporting
units are not required to use this template, but may submit equivalent information
in an alternate format.
Compatibility and Alternate Formats
This file was created in Microsoft Excel version 2003 and may not be compatible
with earlier versions of Excel. To view this file, you will need either:
- An updated version of Microsoft
Excel.
- An updated version of Internet
Explorer.
OR
- A version of the Microsoft
Viewer for Excel compatible with your operating
system.
The OSU Web Accessibility Center provides this template in several alternate
formats, including:
- Accessible HTML version of the MWAS Reporting
Template.
- Acrobat Forms version of the MWAS
Reporting Tempate (requires either Adobe
Reader or Adobe
Acrobat version 6.0 or above).
How to Use The Excel File.
You can use this Excel file to input and track your progress and prepare your
annual report. Fields not requiring input have been locked. You will only be
able to edit the input boxes. To request an unlocked version of this document,
e-mail the Web Accessibility Center at webaccess@osu.edu.
STEP 1 -- MWAS Report PT1.
Start with the worksheet called "MWAS Report PT1" and fill-in as much information
as possible. For the "Strategies" section, you will need to select "Yes" or "No" from
the drop down boxes.
STEP 2 -- MWAS Report PT2.
Next, move to the worksheet titled "MWAS Report PT2." Here, you will input
the majority of the required statistical data. If you are unclear what is being
asked, consult the worksheet called "MWAS Report Key" for clarification and
definitions. You also have space to enter comments, addendums, and other notes
about issues that impact the report (including excluded pages/folders/areas
of your site). Note that you do not need to re-enter your department information;
it is brought forward from the first worksheet.
STEP 3 -- Print Part 1 and Part 2.
You will need to print both the MWAS Report PT1 and MWAS Report PT 2 worksheets.
Together, they make-up the full MWAS Report.
